Form Import with AI

Have you been working with your own paper assessment form for years and don’t want to rebuild it field by field? You don’t have to. Upload the form as a PDF or photo – the AI recognizes sections, fields and answer options and creates a digital form from them. You review the suggestion in the form editor and only save once everything is right.

How It Works

  1. Open forms

    Go to Settings → Forms. There you will find the sections Findings forms and Anamnesis forms.

  2. Start the import

    In the section you want, click Import from document.

  3. Upload the document

    Drag the file into the field or click to select it. The AI reads the document – depending on the number of pages, this takes a moment (“Recognizing form…”).

  4. Review the suggestion

    The form editor opens with the recognized form and the AI suggestion notice. Check all fields, tabs and required settings, rename fields, and add or delete whatever doesn’t fit.

  5. Save

    Save the form. From now on it is available when creating an assessment or when sending a medical history form.

Tips for a Good Result

  • Use a blank form: Filled-in content is ignored – only the form structure matters. A blank, clearly legible form gives the cleanest result.
  • Photograph straight on: When using photos, make sure there is good light and a straight angle.
  • Long forms: A maximum of 10 pages is read. For longer documents, split the form up.
  • Some rework is normal: The AI delivers a draft. You can change field types (e.g. selection list instead of text field) in the editor at any time.

Is client data from the form imported?

No. The AI only takes over the structure of the form – entries in the form are ignored. Even so, we recommend uploading a blank form.

Can I change an imported form later?

Yes. Imported forms are completely normal forms in TheraTap and can be edited in the form editor at any time.
